Insurance Information
Mindful Eats Nutrition Counseling accepts private pay and some Blue Cross Blue Shield (BCBS) insurance plans. If you do not have BCBS, we can provide you with a superbill to file with your insurance company for out-of-network reimbursement.
Please be aware that if BCBS denies coverage for an appointment, you will be responsible for the full price of the session; thus, we highly recommend calling to verify your benefits before the first appointment.
Verifying Benefits for Blue cross blue shield
1. Call the member services on the back of your insurance card. The following questions are helpful to ask:
Do they cover procedure code S9470 (nutrition counseling/dietitian visit)? Or 97803?
Does my plan only cover visits that are considered “medically necessary?”
Do I need to meet a deductible first? If yes, how much?
Do I need a physician referral? (Note: All BCBS HMO plans require a physician referral before the first session in order for coverage to be obtained).
Is there a limit to how many sessions are covered?
What is my co-pay amount for outpatient nutrition counseling?
Are there any diagnosis codes that are excluded?
2. We use Z71.3 (Dietary Counseling and Surveillance) when we do not have a diagnosis to submit or if the medical diagnosis you are being seen for is a plan exclusion. Ask if Z71.3 is covered.
3. Ask if there is any difference in coverage for Telehealth versus in-office visits.
4. Obtain a reference number of the call for you to have on file.
